Care with Caroline is a medical care management, nurse concierge service, which provides a private professional advocate and guide for you or family members who want to ensure their loved one is in the best hands.  Care with Caroline provides you or your loved one with the right kind of care at the right time with the goal of extending independent living.

I do this through planning and coordinating care of the elderly and others with physical and/or mental impairments to meet long term care needs, improve quality of life, and maintain independence for as long as possible. This entails working with an individual and their families in managing, rendering and referring various types of health and social care services.

Services

Care with Caroline provides individualized medical services to you or your loved one.  Below are general examples of ways I can help.

Nurse Helping a Patient
Medical Information
  • Evaluating, arranging and monitoring hired caregivers such as home health aides or personal care workers.

  • Coordinating medical appointments, transporting to appointment, advocating during the appointment and communicating the outcome.

  • Identifying social services and programs that could be beneficial. 

  • Making referrals to financial, legal or medical professionals and suggesting ways to avert problems.

  • Act as the medical expert by explaining complex or difficult topics to the client and their family.

  • Creating short- and long-term care plans.

  • Acting as a liaison to families who may be hundreds of miles away.

  • Answering questions and addressing emotional concerns of caregivers.

  • Arranging for relief or respite care for stressed-out caregivers.

  • Medication management such as filling med boxes and coordinating refills.

  • Multi-dose medication pack preparation (Bubble pack/Blister pack cards).

  • Medical escorts.

  • Post operative Care.

  • Supplementing Home Health care services.

  • Handyman services*

When to initiate

Care with Caroline

Knowing when to bring in outside help can be difficult. My services may be beneficial if you are noticing the following.

The person you are caring for:

  • has multiple medical or psychological issues

  • multiple recent hospitalizations

  • is unable to live safely in their current environment

  • is not pleased with current care providers and requires advocacy

  • is confused about their own financial and/or legal situation

  • has limited or no family support

Or if your family:

  • has just become involved with helping the individual and needs direction about available services

  • is either “burned out” or confused about care solutions

  • has limited time and/or expertise in dealing with the individual’s chronic care needs

  • does not live close by and unable to do as much as you wish

  • is at odds regarding care decisions

  • needs education and/or direction in dealing with behaviors associated with dementia

About Me

I am Caroline Lasser RN

I've always had a lifelong passion for helping others. As a registered nurse, I am able to combine this passion with my desire to make a difference. I obtained my Bachelor’s in Nursing at Winona State. After a few years working at Mayo Clinic in Rochester, MN., I went on to specialize in geriatrics and end of life care.

I have over 16 years of experience specialized in caring for elderly individuals in their homes. I have a natural magnetism for "The Greatest Generation". I am  known for my patience, listening ability, and love of puns. I enjoy getting to know my clients and their families in order to care and celebrate each individual to the fullest. 

When not at work, I enjoy reading and spending time with my four children and two dogs. I grew up in Whitefish Bay and still live in the North Shore.
